Registered Nurse- FT Weekends / PRN

Position Summary

The staff nurse is responsible for the organization of nursing operations and floor staff on the assigned unit during the shift and for interface with other departments in providing total patient care.

Essential Duties

  • Maintains familiarity with the condition of each patient on the assigned unit.
  • Maintains accurate and complete medical records to include, but not limited to, incident reports, immunizations, orders, & census.
  • Communicates and documents with physicians and family regarding the patient’s care.
  • Participates in the identification of learning needs for the staff and corrects the performance of the task.
  • Complies with, and holds floor staff accountable with, policies, procedures, rules and regulations of the facility.
  • Maintains a clean and orderly chart room and medication cart.
  • Maintains confidentiality of information about the facility and the individual residents/patients.
  • Makes rounds with physicians (LTC) as needed. Contact physicians/NP/PA with questions/concerns after completing the SBAR form for change of condition as needed. Group non urgent calls to providers 2x/day to avoid multiple provider interruptions.
  • Manages Dr. Call List and follow up from the prior day of completion of items on the list.
  • Verify and correct any orders from physicians throughout the shift.
  • Create and audit lab requisitions ordered and their results are in their respective binder, if not, the charge nurse will make sure it gets placed in the binder or faxed.
  • Round with CNA’s at the beginning and end of their shift, as well as throughout the day to make sure all residents/patients rooms are clean and prepared for the patient.
  • Monitor CNA breaks and lunches to ensure that the breaks are being taken on schedule and no more than one CNA off the floor at one time.
  • Check PCC dashboard for the unit to audit task completion prior to CNA leaving shift and checking through day if floor nurse medications/treatments are being completed.
  • Check dashboard prior to CNA shift ending for alerts on refused or incomplete baths and correct prior to CNA leaving or progress note why patient did not receive a bath, ensuring that they receive one when requested.
  • Provides care for the whole resident.
  • Passes medications, and performs skin/wound treatments as ordered.
  • Includes the family whenever possible in the treatment of the resident.
  • Sets up and administers medications in compliance with standard nursing practice.
  • Uses nursing discretion in providing appropriate treatment for residents.
  • Accurately and promptly documents all aspects of resident care.
  • Reports significant observations to the DON/ADON 
  • Assists in the evaluation of subordinates.
  • Flexible in scheduling and volunteering for overtime as needed.
  • Review BM list and ensure bowel protocol has been followed and documented.
  • Handle family concerns and escalate to DON/ADON as needed for follow up and resolution.
  • Complete detailed 24hr report to include, laxative list, INR, skin issues, admissions/discharges, and any documentation needed for communication as requested.
  • Walking daily rounds to each resident/patient to ensure their needs are being met and they are satisfied with their care for the day
  • At the beginning and end of the shift the charge nurse and floor nurse will conduct report with oncoming shift then communicate with CNA’s the needs/expectations for the day.
  • Responsible for patient discharge, verifying packet is complete, medications are given and signed for, Sunny Vista equipment does not go home with patient, DME ordered for discharge is available if applicable, and vendor is notified to return any rental equipment that was obtained for the patient during their stay as requested.
  • Other unexpected duties that arise through the day not defined above.

Basic Qualifications & Experience

  • Current licensure as a Registered Nurse or Licensed Practical Nurse in the State of Colorado.
  • One year experience as a RN or LPN in a long term care facility.
  • Ability to organize work for accuracy and promptness.
  • Good knowledge of resident care.
  • Strong management skills.
  • Aware of the Nurse Practice Act and the individual responsibility for resident care.
  • Must be able to bend and stoop as well as push, pull and/or lift a minimum of fifty pounds.
  • Responsible for obtaining a replacement if unable to work a scheduled shift.

Pay Range Minimum: $44.29 per hour; Maximum:$48.40 per hour.  The base range represents the low and high end of the pay range for this position. Actual pay will vary and may be above or below the range based on various factors including but not limited to location, experience, and performance.
