Social Services Director

Position Summary                                                         

 Responsible for the day-to-day operation of the Social Services in assigned community, working directly with residents, family members and coordinating care and support between interdisciplinary team.

 Essential Duties                                                       
  • Assess, evaluate, and strive to meet the psychosocial needs of the residents.
  • Assists in the preparation of the Social Services budget and maintains compliance with budgetary guidelines.
  • Provides information and referral to residents, families and staff that link individuals to internal and/or external services.
  • Disseminates information regarding available financial resources and helps maintain and ensure continuation of any monetary benefits that residents are entitled and/or eligible.
  • Mediates disputes between and among residents, families, and staff.
  • Assists with crisis intervention and assists with appropriate individuals to develop alternative coping strategies.
  • Designs, implements, and evaluates programs and services that that serve individual needs and interests.
  • In compliance with the Company’s policies, if applicable, is responsible for interviewing, hiring, developing,       supervising, and following the Company’s performance management policy with direct reporting staff.

 Basic Qualifications & Experience

  • Must be a licensed LCSW
  • A minimum of two (2) years’ experience in long-term care preferred.
  • 1-year experience in a supervisory capacity.
  • Experience with computer word processing and database programs.
  • Must be able to read, write and speak the English language.
  • Must possess excellent customer service skills
  • Light physical effort required by handling objects up to 20 pounds occasionally and/or up to 10 pounds frequently.
  • Sits, stands, bends, and moves intermittently during working hours.
  • Other duties as assigned.

6 CCR 1011-1 Chapter 5 Section 10.2 Social Services Staffing

10.2 STAFFING The facility shall employ social services staff that is qualified according to the criteria below and sufficient in number to meet the social and emotional needs of the residents.

A) A qualified social work staff member is a person who is either:

1) A social worker with a Bachelor’s degree in social work registered or authorized expressly by Colorado law to practice as a social worker who has one year of social work experience under the supervision of a licensed social worker in a health care setting working directly with residents, or

2) A social worker with a Master’s degree in social work who is licensed as a licensed social worker or licensed clinical social worker under Colorado law, or

3) A person with a Master's or Bachelor's Degree in a related human services field who has monthly consultation with a person meeting the qualifications in subsection 2 above. The consultation shall be sufficient in amount to assist the social work staff to meet resident needs.

 

B) Effective June 1, 2016, any facility that has employed a person with a high school degree or GED as social services staff may continue to employ that individual as long as the facility can document consultation as prescribed in section 10.2(A)(3).
